VOLUNTARY AND CONFIDENTIAL REPORTING SYSTEMS (VCRS)
1. Objective of the reporting system
- The key objective of TIACAO's voluntary and confidential reporting system is to enhance the ATS safety activities through the collection of reports on actual or potential safety deficiencies that would otherwise not be reported through other channels.
- Such reports may involve occurrences, hazards or threats relevant to the ATS safety activities.
- This system does not eliminate the need for formal reporting of accidents and incidents according to ATSOM, as well as the submission of mandatory occurrence reports to the relevant ANS Safety Standard Department.
- The system is a voluntary, non-punitive, confidential reporting system administered by TIACAO, providing a channel for reporting aviation occurrences or hazards while protecting the reporter’s identity.
2. The scope of reporting system
- Aircraft deviation without clearance
- Significant weather information
- Radio Telephony discipline
- ATS Logbook/position log book
- Traffic/separation planning
- CNS operating status
- ATS unit/working environment
- Procedural, standards of CAAN
3. Who can make a voluntary report
- Flight and cabin crew members
- Air traffic controllers
- Licensed aircraft engineers, technicians or mechanics
- Employees of CNS maintenance
- Ground handling agency
- Aerodrome employees
- General aviation personnel
4. When to make such a report
- You wish for others to learn and benefit from the incident or hazard but are concerned about protecting your identity
- There is no other appropriate reporting procedure or channel
- You have tried other reporting procedures or channels without the issue having been addressed
5. How the reports are processed
- All reports are processed with attention to protecting the reporter’s identity.
- Every report is read and validated by the manager, who may contact the reporter for clarification.
- Once complete, the manager de-identifies and enters the data into the VCRS database.
- If needed, only de-identified data is shared with third parties.
- The VCRS form is returned to the reporter with the return date annotated.
- Reports are processed within 10 working days unless further discussion or third-party input is required.
- If the manager is unavailable, the alternate manager will handle the report.
- Every VCRS report will be reviewed and followed up by the manager or alternate.
6. Safety information sharing
- De-identified reports may be shared within TIACAO and with external aviation stakeholders as appropriate.
- If a report reveals an immediate safety threat, it will be prioritized and referred to relevant authorities after de-identification.
